Please sign to agree to the group class policies:

- If a class, workshop, or program is cancelled within the 24 hour time period prior to the scheduled date, there will be a cancellation fee of 50% the full rate.

- If participants choose to end the class, workshop, or program early for any reason, they will forfeit the remaining time of the session, will not receive a refund and will not be able to reschedule that class, workshop, or program.

- Participants are partaking in all exercise recommendations at their own risk. Participants understand that this class, workshop, and/or program is not meant to diagnose or treat any illness, disease, or any other physical or mental disorder, injury, or condition. Elyse and Sparkes Wellness LLC are not responsible for any injury occurring during or after the class, workshop, or program.

- Participants will inform Elyse about the state of their health, and will transmit to her any recommendations and restrictions on the part of their medical doctor or therapist insofar as this coaching is concerned.

- Results vary greatly person to person and no specific results are guaranteed.
